Italy ... Right ?

I don't know which is worse, the "thorn" idea or the fact that the peninsula is facing the wrong direction, probably the latter.

Er... The French do know what Italy looks like, it is rather close by.
Maybe the tatooer was showing the work in progress in a mirror and the guy just didn't think twice.

it says above"La Spina" i guess...and i also guess it might be his surname...
spina usually is translated as thorn...so everything should be explained...well not really!!!
i m italian and i would ever never get a shit like that on me!!!

i also know a guy who got done a "Slipknot" logo on his sheen in the same wrong way with the explanation :i wanted wrong way so any time i look at myself in the mirror it s the right way...the tattoo is mine and i ve to enjoy mainly the sight of it!!!"

I just used Google translate to know what 'La Spina' meant, and it said, "The Plug".
